These Terms and Conditions set forth the general terms and conditions for the sale, issuance, rules and use of the voucher ordered at www.wazektomia.com.
2. Definitions2.1. Seller – Centrum Planowania Rodziny sp. z o.o. with its registered office in Warsaw (02-999 Warsaw, ul. Kazachska 5/U1, REGON: 524501334, NIP: 9512562125) registered in the District Court for the Capital City of Warsaw, Division of the National Court Register under the KRS number: 0001020352
2.2. Voucher – a voucher sold and issued by the Seller, entitling the Holder to one vasectomy procedure performed by the Seller.
2.3. Purchaser – a natural person who purchases a Voucher on the Website.
2.4. Holder – the person redeeming the Voucher, whose data appears on it.
2.5. Terms and Conditions - means these Terms and Conditions;
2.6. Website – a website where Vouchers are offered for sale , operating under the domain www.wazektomia.com.
3. General provisions3.1. These Terms and Conditions are an integral part of the sales contract for the products offered by the Seller and at the same time define the terms and conditions and rules of sales conducted by the Seller.
3.2. Each Purchaser purchasing a Voucher is required to familiarize themselves with these Terms and Conditions.
4. Subject of sale4.1. The subject of the sale is the Voucher defined in item 2.2.
4.2 The price of the voucher is expressed in Polish zloty and includes VAT.
4.3. Payment for the order completion is made on-line via Paynow with the selected payment method.
4.4. The Seller is not responsible for incorrect data of the Purchaser or Holder indicated by the Purchaser when placing an order.
5. Order processing5.1. Product orders can be placed through the Website.
5.2. Providing personal data in the order form is voluntary, although necessary for the implementation of the sales contract concluded with the Seller. The Purchaser and the Holder have the right to access and rectify the content of their data.
5.3. To purchase a Voucher, select a product and make an order.
5.4. After a properly placed order, confirmation of the contract conclusion will be sent to the Purchaser at the e-mail address indicated by the Purchaser when placing the order, and the data for making the payment will be additionally displayed on the Website.
6. Delivery6.1. The Voucher will be delivered in physical form to the Purchaser by courier to the address provided during the order.
6.2. The term of the sales contract execution is from 1 to 5 working days.
7. Voucher redemption7.1. As a condition for using the Voucher, the Holder must schedule an appointment for the vasectomy procedure to which the Voucher relates.

7.3. Currently available dates are published on the Website at https://www.wazektomia.com/#terminy
7.4. The appointment is considered scheduled upon receipt of confirmation of such fact from the Seller.
7.5. The Voucher can only be used once.
7.6. The expiration date of the Voucher is 1 year from the date of purchase.
7.7. Voucher payments cannot be combined with other promotions and discounts.
7.8. The voucher is not exchangeable for cash.
8. Personal Data Processing8.1. The Seller is the personal data controller.
8.2. The data will be processed for the purpose of redeeming the Voucher, the establishment and investigation of possible claims and the performance of the Seller's legal obligations under tax or accounting regulations.
8.3. The data will be kept for the period necessary for the performance of the contract and post-termination settlements and related legal obligations of the Seller (for accounting purposes) and for the period necessary to establish, defend or pursue possible claims.
8.4. The Purchaser has the right to request access to their personal data, rectification, deletion, restriction of processing and portability. The Purchaser has the right to lodge a complaint to a supervisory authority and to withdraw consent at any time without affecting the legality of the processing carried out on the basis of the Purchaser's consent before its withdrawal.
8.5. The Purchaser has the right to object to the processing of personal data on the basis of the controller's legitimate interest, as well as to object to the processing of the Purchaser's data to the extent to which the Purchaser has previously given their consent.
8.6. In order to exercise the Purchaser's rights, related to the processing of personal data, the Purchaser may contact the data controller at rejestracja@wazektomia.com.
9. Right of withdrawal - returns9.1. A purchaser who is an individual, in accordance with the Law on Consumer Rights, may withdraw from the Voucher purchase agreement within 14 calendar days, provided that the Voucher has not been used within that time. In order to withdraw from the contract, the Purchaser submits a statement of withdrawal and sends it by email to rejestracja@wazektomia.com.
9.2. Refunds are made within 14 days of receiving the statement of withdrawal from the purchase.
10. Complaints10.1. How to make a complaint:
Complaints regarding the purchase of a voucher or its redemption can be submitted:

- in person at the doctor's office at the address: Centrum Planowania Rodziny ul.Kazachska 5 lok.U1 02-999 Warszawa
10.2. Content of the complaint:
The complaint should include:
- the name of the person making the complaint,
- order number or voucher number,
- a description of the problem and the expected way to solve the case,
- contact information (e.g., e-mail or phone number) in order to respond.
10.3. Deadline for processing complaints:
Complaints are processed within 14 days of receipt. A response to the complaint will be sent to the e-mail address provided or by any other means indicated by the user.
10.4. How to handle complaints:
If the complaint is found to be valid, the voucher may be:
- replaced with a new one,
- cancelled with the possibility of a refund,
- provided with another form of compensation agreed with the Patient.
11.1. In matters not covered by these Terms and Conditions, the provisions of the Civil Code and other laws shall apply.
11.2. The court competent to resolve disputes that may arise from the application of these Terms and Conditions is the common court with jurisdiction over the registered office of the Seller.
